The Family Practice & Counseling Network (FPCN) provides comprehensive primary care, dental and behavioral health services. The Network consists of three (3) federally qualified health centers (FQHC) in Philadelphia:

  • Abbottsford Falls Family Practice & Counseling (Northwest Philly);
  • Stephen & Sandra Sheller 11th Street Family Health Services of Drexel University (North Philly); and
  • Health Annex (Southwest).

Today the FPCN serves more than 20,000 patients annually and is a NCQA certified Patient-Centered Medical Home and recognized by HRSA as a National Quality Leader for Behavioral Health.


POSITION SUMMARY

The Clinical Nurse is responsible for but is not limited to: patient triage, patient assessment, patient education and counseling, and supervision of medical assistants. The Clinical Nurse ensures timely and productive patient flow within Primary Care and connects patients to services as needed. This position requires a RN.


ESSENTIAL DUTIES

  1. Assure that all operating procedures and regulations are followed.
  2. Develop and coordinate required medication administration practices and processes.
  3. Knowledge of psychotropic/addiction/IDD medications and potential side-effects.
  4. Professionally represent RHD at National Nursing, County, and State forums, workshops and conventions as requested by the Program or Company leadership.
  5. Assist with developing and conducting staff training programs for other nurses and new hires.
  6. Ability to operate with minimal supervision in residential, clinical or community-based environments.
  7. Ability to conduct and coordinate healthcare meetings and trainings for the individuals and staff.
  8. Assist with the development and implementation of policies and procedures for the medical aspect of the designated program.
  9. Assures quality of care by:
    1. adhering to therapeutics standards;
    2. measuring health outcomes against patient care goals and standards;
    3. making recommendation for necessary healthcare adjustments;
    4. Following standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations.
  10. Documents patient care of services by charting in department records.
  11. Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; and establishing professional networks.
  12. Maintains a cooperative relationship among health care teams by communicating information; responding to request; and participating in team problem-solving methods.
  13. Maintaining an effective recordkeeping system as it relates to all forms, and publications.
  14. Maintain an effective tracking and filing system.
  15. Utilize Microsoft Word, Excel (create tracking sheets), and Power point to complete report.
  16. Expected to engage individuals in a trusting, supportive and professional relationship, while providing emotional support within professional boundaries.
  17. Meet established productivity requirements set by the program guidelines.
  18. Maintain professional and timely treatment communication and documentation.
  19. Other duties as assigned

Clinical Care Management:

  1. Assume leadership role in the health center's participation in the Chronic Care Collaborative, Phase 2 including ownership of monthly reporting
  2. Attend all learning sessions, webinars, and other mandatory participatory calls as required by the Chronic Care Collaborative
  3. Develop a Referral Tracking System for the Primary Care Department
  4. Develop a Hospital Discharge Tracking system, making contact with local facilities to establish better communication from hospital to Primary Care Provider
  5. Receive and analyze all reports from insurance providers or area hospitals that give us information that patients have been hospitalized or have utilized an Emergency Room Contact all patients in the above category within 3 business days of learning of their hospital discharge to arrange for needed care management
  6. Receive, review, and make recommendations regarding Focused Care Management Referral Forms submitted by providers of care in the Primary Care, Behavioral Health, and Dental Departments
  7. Work with other Care Management Team members to review and accept patients into the Care Management Program
  8. Plan and implement strategies to reduce resource consumption and achieve positive patient outcomes
  9. Monitor Care Management Program and report to team areas that need focus and re- work processing to achieve better outcomes, developing proposals to achieve needed change
  10. Maintain a log of all patients entered into the program and key information related to each patient for data collection purposes
  11. Submit reports as appropriate and necessary for grants, collaborative and annual reporting purpose
  12. Function as a resource to clinic staff regarding patient assessment and plan of care
  13. Analyze aggregate data, address deviations from program policies, and identify potentials for growth and improvement of the program based upon individual patient responses to the program and overall patient progress as a result of the program then share this information with staff and administration.

QUALIFICATIONS

  • Bachelor of Nursing Degree from an accredited institution required.
  • Current unrestricted Registered Nurse with appropriate State License; Commonwealth of Pennsylvania required.
  • At least 5 years' experience as a RN required.
  • Experience in an outpatient practice setting preferred; experience working within OB for pre-natal nurse.
  • 5-7 years professional nursing experience required at least two of which working with patients with intellectual developmental disabilities or behavioral health issues preferred (RHD).
  • Current CPR Certification
  • COVID Vaccination
  • Previous experience working with an Electronic Health Records database.
  • Knowledge of primary health care and the social determinants of health
  • *Knowledge of concepts of health promotion, disease prevention, behavior change counseling, program planning, individual and group counseling.
  • Nursing care knowledge related to primary care/community health/chronic disease management
  • Excellent organizational skills to manage numerous practice activities.
  • Demonstrates initiative, leadership and a high level of nursing skills.
  • Unconditional ability to maintain HIPAA regulations.
  • Leadership skills that include the ability and desire to champion teambuilding within the Primary Care
  • Demonstrates fiscal responsibility
  • Demonstrates effective time management skills and follow through
  • Ability to communicate professionally in both verbal and written forms.
  • Ability to work independently and manage multiple expectations.
  • Proficient computer skills.

LIFTING REQUIREMENTS

Light: exerting up to 20 pounds of force frequently, and/or negligible amount of force constantly to move objects. The use of arm and/or leg control requires force greater than sedentary, but worker still sits majority of time.

Covid-19 Vaccine Mandate: FPCN, a program in the City of Philadelphia, are required to follow the COVID vaccine mandate for healthcare workers per Philadelphia Department of Public Health. FPCN requires Healthcare Workers to be fully vaccinated. Fully vaccinated means it has been two weeks since the final dose of vaccine. Thus, we require vaccination prior to start for all new hires. This currently does not include boosters.
