- The ability to provide and assist team members in coordinating the behavioral and physical health care needs of program participants.
- The ability to maintain a record of medical and behavioral health history and on-going medical and treatment records. Review all charts one time per month (approximately 25 charts per week) to ensure medical needs have been met and documentation meets applicable standards.
- The ability to monitor participant's medication. Verify insurance for medication approval by making phone calls to appropriate insurance and pharmacies as directed by psychiatrist.
- The ability to participate in discharge planning for participants from acute or extended care facilities to provide continuity of medical substance abuse and psychiatric care. Coordinate with participant's case manager and therapist for scheduling of appointments.
- The ability to complete required documentation. Develop, review, and revise forms, procedures and standards of care according to accepted professional practice. Conduct quality assurance reviews of health care section of medical records.
- The ability to provide educational training services to participants, provider agencies, and non-medical staff, including medication, diagnosis and interventions training classes
Scope:
- Frequent talking and listening.
- May require verbal crisis intervention assistance in emergency situations.
- Must be available for local travel and possess a valid driver's license with an acceptable driving record.
- Scheduling flexibility and possible on-call work are necessary.
- May be required to report to work during emergencies including inclement weather.
