Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice. We are seeking a part-time Clinical Specialist to provide individual and group therapy to program residents utilizing either of two evidence based practices: Dialectical Behavioral Therapy (DBT) or Cognitive Therapy for Psychosis (CT-R).
Additional Responsibilities include:
- Lead skill building groups consistent with DBT model.
- Conduct family therapy, as needed, for residents' identified family.
- Provide clinical feedback to milieu staff on interventions and responses to residents.
- Maintain a safe living environment, utilizing crisis management, for all residents.
- When applicable, to provide supervision to Residential Staff including Case Management.
- Complete and maintain all chart documentation according to insurance and licensing regulations including progress notes, treatment plans, concurrent reviews within established timeframes.
- Assure each resident has input into developing an up-to-date treatment plan; modify plans to adapt to special needs and/or service delivery gaps.
- Establish an on-going dialogue with other members of the rehabilitation/treatment team, including residential staff, case managers, vocational/educational instructors, consultants, members of the medical profession and family members, as needed, to provide continuous supports; schedule planning and linking conferences as needed.
